Percutaneous coronary intervention (PCI) by Rotational atherectomy (Rota)　in severe calcifiedlesion followed by Drug Eluting Stent (DES) is effective. Recently Drug Coated Balloon(DCB)for small vessel showed favorable results. However, results of Rota and DCB in non-small calcified lesions are not clear.

Between January 2016 and Aug 2018,472 calcified lesions were treated with Rota.We exclude reference vessels diameter less than 2.5mm by QCA .104 lesions of Rota+ DES group and 80 lesions Rota +DCB group were extracted. Clinical and procedural data of those case were retrospectively analyzed ．The primary endpoint was MACE at 1 year that were defined as cardiac death , non-cardiac death , target vessel related myocardial infarction or target lesion revascularization(TLR).

Clinical follow up was performed 92% in Rota+DES and 98% in Rota+DCB. MACE at 1 year of Rota + DES and Rota + DCB were（8% VS　11% ,P = 0.30)  in terms of cardiac death  ( 0% VS 0%), non-cardiac death (4% VS 3%, P=0.36), target vessel related myocardial infarction (0% VS 0%) and TLR  (4% VS 8%, P = 0.30 ).

After Rota +DCB, clinical outcomes at 1 year was comparable to Rota + DES in non-small calcified lesions. Rota+DCB treatment going to be a new strategy．
